Full subscription for calendar year £10.00 (Juniors £5.00)
Associate members £10.00 (Juniors £5.00)
Pool charges for Yeovil and Sherborne are:
Adults £4.00 per swim
Juniors £1.70 per swim
Associate members £4.00 per swim
Associate juniors £1.70 per swim

Please print the form, complete page 1 and ask a Healthcare Professional to complete page 2, and send the completed form to the address below. (A Healthcare Professional is sufficient. There is no longer any need to have your application form signed by a doctor)
